Driver’s License Vision Test Options, Requirements

Vision Test Option A
Present a certificate of eye examination completed by a licensed ophthalmologist or optometrist within six months
of your driver’s license application/renewal.
Upon a patient’s request, eye doctors on Oʻahu can issue a vision certificate for a driver’s license
application/renewal that includes the patient’s name; date of birth; date of examination; visual acuity for the right
eye, left eye, and both eyes; whether corrective glasses or contacts are required to drive; peripheral vision check;
and the signature of your licensed ophthalmologist or optometrist.
You must submit the original certificate that is signed by your eye doctor and dated within the previous six
months; you might want to make a copy of the vision certificate for your records.
A prescription for corrective glasses or contacts is not a vision certificate and will not be accepted for the driver’s
license vision test requirement.
Vision Test Option B
Take a vision test during the driver’s license application/renewal process at the driver’s license center or
designated satellite city hall that offers Hawaiʻi driver’s license renewal services.
To address concerns about high-touch public areas, City and County of Honolulu driver licensing centers and
designated satellite city halls offer individually wrapped alcohol wipes to customers who would like to wipe down
the apparatus before taking a vision test.
Vision Requirements: Minimum standards of vision to qualify for a Driver’s License*
20/40 or better vision in one eye, corrected or uncorrected.
Peripheral vision of 70 degrees or more using one eye.
If you have corrective lenses, either glasses or contacts, the “corrective lenses must be worn” restriction
requirement will be noted on the back of your driver’s license.

*Applies to Instruction (Learner’s) Permit, Provisional License, Moped License (Class 1), Motorcycle License (Class 2) and
Driver’s License (Class 3). Additional minimum standards apply to the Class 4 Driver’s License for larger noncommercial
vehicles and all Commercial Driver’s License (CDL).
